Eligibility for application

• Applicants: patients, family members or proxies.
• For patients:

  Power of Attorney : Download
• After completing the above documents, mail to m963@mail.cmuh.org.tw.

Location for obtaining medical records

Please go to counter no. 6 of the admission center on the 1st floor of Lifu Medical Building to obtain the medical records by producing your documentation.

Workdays

• Copies of medical records and reports: 1 workday, not more than 3 workdays at the latest.
• Copy of complete medical records: 3 workdays, not more than 14 workdays at the latest.
• Medical record summary in Chinese: 14 workdays.

Charge

• Copies of medical records and reports: basic fee (one copy) is NT$200, with NT$5 for every extra one.
• Medical record summary in Chinese: NT$500 per department, with 7-14 workdays.
• CD: NT$200 for one single test; NT$250 for 2 tests; NT$300 for 3; NT$350 for 4.
• Copies of examination report in color and trauma photo: basic fee NT$200, plus NT$40 for each extra page.
• NT$200 for each X-ray film.
• CD: NT$200 for one single test; NT$250 for 2 tests; NT$300 for 3; NT$350 for 4; NT$400 for 5; NT$450 for 6; NT$500 for 7 and more (with NT$100 for each additional CD).
• Mailing charge: NT$50 for prompt registered mail and NT$120 for express mail.
