Documentation required for application：

(1) registration card and NHI IC Card.

(2) Original identity card of the applicant and his/her proxy (for children, please present household certificate) or passport (for foreigners).

The patient may also submit all required documents to the inpatients reception desk and file out the application through the reception desk for free. If the newborn has left the hospital, the parent shall make an obstetric appointment to apply for the certificate between 8:30 and 17:00, Monday to Friday. Parents are required to present Chinese birth certificate, their own passports, as well as the newborn’s passports.
Death certificate (Chinese)	$30	Fill out by spouse, other immediate family members or an entrusted agent.	the same day as the application	The family of the deceased must request a certificate from the attending physician. After which a payment of the prescribed fee is required. The family can then collect the certificate at the cashier counter with the proof of the deceased’s ID.
