Instructions on Admission/Discharge Procedures

Precautions for Admission
Please report to the Admission Center personally on the day of hospitalization.
Please bring the following information when you report to the hospital on the day of admission :
- 1. The patient’s original ID card and NHI IC Card.
- 2. Those who are to undergo surgery must bring the completed consent form.
- 3. Please bring your own daily necessities, and for those accompanying the patient, please bring your own blanket and pillow.
- 4. Please bring along daily medication or tell nursing staff its name.
- 5. Do not to bring valuables such as earrings, rings, necklaces or cash.
- 6. Please take your mobile phone with you and keep it carefully.
To turn finite life to infinite love, the Hospital, in line with national health policy, provides the "Consent Form for Organ Donation" for all inpatients to consider, as a basis for organ removal, which allows family members to fully understand the wishes of patients (Extension: 1764 or 1462).
Discharge Procedures
You may apply for hospital discharge of your own free will. However if the physician does not recommend you to do so based on your condition and you still insist, you and your family should sign the “Voluntary Discharge Statement” to apply for discharge in accordance with the provisions stipulated in Medical Care Act.
Regarding discharge application, the nursing station will hand you the "discharge notice," "discharge prescription," and "discharge appointment registration note." Please present the NHI IC Card, "discharge notice," and "discharge prescription" at the 1F admission center for processing.
Before discharge from hospital, the medical staff should inform the patients of precautions for self-care after discharge, clinic revisit or transfer, in addition to providing follow-up care information as far as possible.
Patients are advised to be transferred to another hospital, with a completed summary of the medical record given to them in case of inability to determine the cause of disease or administer complete treatment due to limitations in equipment and expertise. However for patients in critical condition, the Hospital will still provide proper first-aid treatment before referring them.
The hospital will notify you to leave the hospital and assist you with referral when you are diagnosed as being able to be transferred to another medical institution for continuing treatment and care. You may continue to receive care at hospitals or institutions at proper care level by presenting the referral note and summary of medical records issued by the Hospital.
To apply for a refund, please bring along your NHI IC Card and original receipt. For a refund on your credit card, please bring the original credit card signature note and original credit card used for transaction for refund processing.
